I collect and keep your completed Consultation Forms (including email, phone numbers, emergency contact number and information about your health). I collect and keep your completed Consultation Forms for KCR/or any other treatment or therapy that I do.

 

When do I collect information?

I collect information from new clients when they complete a consultation form and have a treatment session.

 

How is it processed?

I read completed forms to be aware of medical or health conditions before any treatment can be given or received.

• To ensure any and all treatments provided by Dell - at Ahtmann Therapies London regarding your health and well being are suitable

• To maintain our own accounts and records regarding your treatments (these are securely stored away)

• To inform you of news, events, activities, and services being offered by Ahtmann Therapies London.

 

Where is this information sent/stored?

Your consultation forms are stored securely in my home, under lock and key.

 

 

How long do I hold your personal data?

The recommendation from my public liability insurance provider is that I keep records for a minimum of 7 years.
